We are pleased to invite you to attend the 8th Annual Wake Forest University Football Coaching Clinic March 18-20, 2010.

Our Clinic will cover a variety of fundamentals and techniques geared to all levels of play. You will have an opportunity to meet and discuss all aspects of football with our staff on an individual basis. Our staff will not only discuss the fundamentals and techniques they teach, they will also instruct you on the drills they use during our “Coaches Coaching Coaches” segment of the clinic. We look forward to meeting you in March at this clinic.In accordance with the rules set forth by the Mississippi State Football Camps, I hereby give my consent for the camper listed below to participate in activities of this

camp, to include this specific sport activities and recreational activities conducted during the camp. The undersigned camper and parent/guardian understand that the

camper will engage in physical activity during the program which contains inherit risk of physical injury and the undersigned assumes the risk, indemnities, and releases

Mississippi State University, its football camps, officers, directors, agents, and employees from any and all liabilities for personal injury arising out of the camper’s

participation in the camp program. If at any time it is necessary for the camper to receive outside or professional medical attention, I hereby give my consent to the camp

to secure the services and arrange transportation if deemed necessary. I am aware that the camp’s medical insurance providers benefits up to $5,000 accidental death

and dismemberment and $10,000 medical expenses with a $100 deductible for which the camper is responsible. There is also $1,000 limit on dental insurance. I am also

aware that I will be responsible for all medical expenses resulting from sickness or any other non-camp related injury or illness.
