The Boys and Girls Club of Lodi, its owners, operators, instructors, employees, agents, servants and affiliated center(s) from any and all liability or in any way related to my or my child’s use of the facilities, equipment, or apparatus of Explore Science, Inc. and The Boys and Girls Club of Lodi; and/or my or my child’s participation in any class, program competition or other event organized, run and/or sponsored by or held at The Boys and Girls Club of Lodi/Explore Science, Inc., hold harmless the said claims demands, costs, expenses and compensation arising out of or in the course of or in any way related to any personal injury to me or my child. I agree that Explore Science Inc. may photograph and/or videotape me or my child during the program and that Explore Science Inc. retains the rights to use these visual images in any manner without compensation to my child. In the event of an accident I would like my above mentioned child to be taken to a hospital for medical treatment and I hold Explore Science, Inc. and The Boys and Girls Club of Lodi and its representatives harmless in their execution of this action. Additionally, I hereby agree to individually provide for all possible future medical expenses which may be incurred by my client as a result of any injury sustained while participating at or for Explore Science Inc. and The Boys and Girls Club of Lodi. Explore Science, Inc. and The Boys and Girls Club of Lodi are not to be held responsible for any injuries that occur while students are not under the direct supervision and care of the employees of Explore Science Inc. We reserve the right to terminate any session without refund for any child that acts in appropriately, causes harm or the threat of harm to themselves or to others, or does not follow the rules of the program/instructor.All terms and conditions are subject to change without notice. Signing confirms to agree to all policies, rules and regulations. I have read the Release of Liability for Personal Injury and have been given the opportunity to speak with a representative of Explore Science, Inc. and The Boys and Girls Club of Lodi before signing this release. I hereby certify that I am a Parent/Guardian of the above child.
