*  Complete registration form (online or download application to register by mail) and pay online with credit card or e-check, or by personal check by mail.

          Personal checks will not be accepted after June 1st – Cash, money order or certified bank check only!

    * Sign and date the waiver of liability and holds harmless agreement.

    * Complete physical or physician statement.

    * Any cancellations will accrue a $100 non-refundable administration fee. No refunds will be made after June 1st.

    * Complete registration form (online or download brochure and application to register by mail) and pay online with credit card or e-check, or by mail with school or personal check.

          To pay by check online, select the “e-check” option.

    * You will receive a confirmation email once you have registered. If you do not receive the email confirmation, you are not registered. Contact the basketball office with questions.
