APS has updated their physical forms as of April, 2010 to add a discussion of Concussion Managment. The updated English version of the form is available per the instructions below.

APS requires a physical of all student athletes prior to participating in any sport. Physicals must be dated on or after April 1 to be current for the subsequent school year (i.e. April 1, 2010 for 2010-2011) and remains current until the first day of practice for fall sports of the following year.
