I am aware of the risks, hazards and inherent dangers that may arise due to my child's participation in the 2010 Univesity of Akron Zips Basketball School and Zips Individual Instruction/offensive Skills Camp for Advanced Players being held at the University of Akron (collectively referred to as "UNIVERSITY") on the following dates: June 21- 24, 2010, July 26-29, 2010 and July 12-15, 2010.
