In consideration for my participation and/or for my child’s participation, I hereby waive, release and discharge any and all liability claims arising out of or connected in any way with my and/or my child’s participation in this activity including damages for death, personal injury or property damage which I may have, or which hereafter accrue to me, against the State of California, SONOMA State University, it’s trustees, officers, employees, students and volunteers, and any public agencies.

I understand that participation in this activity may pose a risk of physical injury to myself and/or my child. Notwithstanding this risk, foreseeable or not and/or resulting from negligence or carelessness on the part of persons or agencies mentioned above, I fully agree to assume such risk. It is further understood and agreed that this waiver, release and assumption of risk to be binding on my heirs and assigns.
